One guess is that the bullet slammed into the rear of the barrel/chamber getting pushed back into the case as it loaded, then when it was fired there was too much pressure too quick. Same as when someone uses a Galil 223 mag in a Valmet 223.

I agree Gray.
The same was happening to mine before repairs. Fortunately, mine did not blow up. It did, however, push the bullet down into the casing.
